Job Description:
As a Patient Care Coordinator with Optum Home & Community Care, you will be part of the Senior Community Care (SCC) product team. Our team works together to provide care to patients at home, in nursing homes, assisted living facilities, or senior housing. This role is a great opportunity to make a difference in people's lives and improve access to care.

We are committed to creating a seamless health journey for our patients across different care settings. As a member of our interdisciplinary care team, you will support nurse practitioners (NPs) and physician assistants (PAs) by assisting with care coordination activities, administrative tasks, and supporting patients and providers.

This full-time position involves working Monday through Friday, 8:00 am to 5:00 pm, with flexibility to work any 8-hour shift schedule during normal business hours. Occasionally, overtime may be required due to business needs. The Field Based Patient Care Coordinator will cover markets including Lucas, Lorain, Sandusky, Ottawa, Huron, and Erie County in Ohio.

We offer two months of paid training, with schedules based on your start date. During this time, you will learn about our company culture, policies, and procedures. You will also receive guidance from experienced colleagues and have opportunities to ask questions and seek feedback.

Key Responsibilities:
• Supporting the interdisciplinary care team with overall member coordination under the direction of an RN or NP/PA
• Data entry and member chart management for new members, post-hospital visits, quality, labs, and medications
• Obtaining medical records following hospitalizations or physician appointments
• Scheduling member transportation or appointments
