Key Responsibilities:
This position involves delivering patient care through systematic data collection and assessment. The Pharmacist identifies and prioritizes patient needs, evaluating the effectiveness of care provided. Responsibilities include:
- Collaborating with the interdisciplinary care team to conduct thorough patient assessments, analyze data, create care plans, implement treatments, and evaluate their effectiveness.
- Administering medications safely in accordance with state regulations and organizational policies.
- Serving as the primary contact for coordinating patient care across various hospital departments.
- Advocating for patient safety by engaging in continuous quality improvement initiatives.
- Processing medication orders, ensuring appropriateness, safety, and efficacy, while performing necessary clinical support duties.
- Preparing compounded and non-compounded products accurately, adhering to safety regulations.

Qualifications:
- Graduation from an ACPE accredited School of Pharmacy with a BS or PharmD (required).
- Advanced pharmacy degree - Doctor of Pharmacy, Masters, or Residency (preferred).
Licensing & Certifications:
- Current pharmacist license in the practicing state(s) (required).
- Valid state-issued ID (required).
- BLS Basic Life Support certification (preferred).
- Immunization certification (preferred).
Experience:
- Minimum of 2 years of recent experience in an acute care environment (required).
- 1 year of leadership experience (preferred).
- Experience in pharmacy practice (required).
- One year of experience in a relevant pharmacy practice area (preferred).
Skills & Abilities:
- Monitoring patient conditions during treatment.
- Administering non-intravenous medications.
- Communicating effectively with medical professionals regarding patient care.
- Documenting patient medical histories accurately.
- Demonstrating financial acumen and budgeting skills.
- Evaluating employee performance.
- Analyzing data and information effectively.
- Exhibiting strong organizational skills and attention to detail.
- Utilizing critical thinking and problem-solving abilities.
- Building relationships and fostering teamwork.
- Managing stress and conflict resolution.
Training:
- Proficiency in EPIC Electronic Health Record (preferred).
- Exhibiting proficient use of all computer hardware and software (required).
