Nursing Care Coordinator

At Sarah Bush Lincoln Hospital System, we are seeking an experienced Registered Nurse to join our team as a Nursing Care Coordinator.
Compensation: The estimated salary for this role is $35.43 per hour, with pay based on applicable experience.
About the Role:
We are looking for a dedicated and skilled nurse who can provide clinical leadership to the patient care team and ensure consistency in care delivery.
Key Responsibilities:
- Analyze assessment data to determine actual or potential diagnoses, problems, and issues.
- Assess patients' conditions and care delivery through direct observation, rounding, and data input from other team members.
- Collect pertinent data and information relative to the patient's health or situation.
- Recognize normal and abnormal findings when gathering data.
- Coordinate bed and patient care assignments during shift and in preparation for the next shift.
- Document and report care delivered to physicians and other team members as needed.
- Enhance professional growth through classes related to communication, teaching, stress management, conflict resolution, and team building annually.
- Evaluate progress toward attainment of goals and outcomes.
- Implement care and supervise care delivered by other team members in alignment with the plan of care and approved safety and infection control standards.
- Knowledge - Actively pursue required knowledge and skills through orientation activities, unit-based in-service programs, reading current literature, and seeking new clinical learning opportunities.
- Outcomes Identification - Identify expected outcomes for a plan individualized to the patient or situation.
- Planning - Develop a plan that prescribes strategies to attain expected, measurable outcomes.
- Provides performance feedback to team members and input to Manager/Director regarding appraisals.
- Relationships - Establish effective working relationships with peers, physicians, and other members of the health care team.
- Serves as a strong, positive role model for other health care team members.
- Teaching - Employ teaching strategies to promote health and a safe environment.
Requirements:
- Active RN in the State of IL.
- ADN (Required).
- ACLS - Advanced Cardiac Life Support within 90 days - Sarah Bush Lincoln.
- BLS or CPR certification within 90 days of hire - Sarah Bush Lincoln.
