Job Summary

Central Carolina Hospital is seeking a skilled Social Worker to join our team. As a Social Worker, you will provide psychosocial care to patients and their families, linking them with community resources and facilitating referrals to treatment and community resources post-discharge.

Key Responsibilities
  • Provide assistance with assuring safety for behavioral health patients, including managing development and delivery of policies, managing staffing needs in coordination with charge nurses and directors.
  • Participate in hospital and community initiatives addressing mental health and substance abuse issues.
  • Fill in as a case manager/discharge planner, providing care coordination and discharge planning for primary medical patients on med/surg floors and in ICU.
Requirements
