- Develop and implement comprehensive service plans, tailored to meet the unique needs of each client.
- Collaborate with treatment teams to provide timely and effective support, ensuring seamless care coordination.
- Document and report client progress, maintaining accurate and up-to-date records.
- May be required to transport clients in an agency vehicle, ensuring safe and reliable transportation.
- Provide on-call services as needed, demonstrating a commitment to client care and support.
- Participate in staff meetings and training sessions, staying informed and up-to-date on best practices and industry developments.
- Uphold confidentiality and maintain the highest standards of professionalism in all interactions.
- Ensure timely completion of administrative tasks, including registration for benefits and authorization for Medicaid services.
- Monitor billing data and units for Medicaid, ensuring accurate and efficient processing.
- Maintain accurate records of client interactions and progress, in accordance with program and contract guidelines.
Requirements:
- Bachelor's degree in Psychology, Social Work, Mental Health, or related field.
- One year of experience in counseling or case management, with a focus on Severely Emotionally Disturbed adults/children.
- One year of experience working with the mental health and substance abuse community.
- Targeted Case Manager certification.
