Job Summary: We are seeking a skilled Care Management RN to join our team at Highpoint Health – Sumner. As a key member of our Wound Care department, you will provide direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.
Key Responsibilities:
- Provide direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Conduct thorough assessments of patients' physical and emotional needs.
- Develop and implement effective care plans to meet patients' needs.
- Monitor patients' progress and adjust care plans as needed.
Requirements:
- Associate's or Bachelor's degree or RN Diploma in Nursing - Required
- Certifications: BLS certification from the American Heart Association (AHA) or American Red Cross (ARC) - Required to be current upon hire
- Licenses: State of Tennessee or Multi-State Registered Nurse licensure – Required
- Wound Care Certification - Preferred
Preferred Qualifications:
- 1 year nursing experience in Medical/Surgical, Homecare or Wound care - Preferred
