The PACE Medical Practitioner collaborates with the interdisciplinary team to ensure holistic care delivery, which is essential in serving our participants effectively.
- Responsible for prescribing suitable medications, health plans, and lifestyle modifications aimed at recovery from health conditions, prevention of future ailments, and promotion of overall wellness.
- Skilled in managing stable patients with chronic illnesses by utilizing internal resources and consultations, ensuring their health is maintained without unnecessary referrals to external specialists.
- Adept at assessing when on-site treatment is appropriate versus when to direct patients to urgent care or emergency services.
- Provides crisis intervention services as necessary, ensuring that behavioral health support is culturally sensitive and compliant with relevant laws and professional standards.
- Facilitates referrals to appropriate Behavioral Health providers, including psychiatrists and licensed therapists, as needed.
- Collaborates with Nurse Practitioners and Physician Assistants, who are vital in delivering primary care to PACE participants.
- Engages in the supervision of advanced mid-level providers as per organizational requirements.
- Oversees family practice and internal medicine residents participating in residency programs.
- Effectively prioritizes daily responsibilities to keep high-acuity patients out of emergency settings and ensure smooth transitions of care.
- Completes all necessary medical documentation in the Electronic Medical Record promptly, adhering to San Ysidro Health Center's standards and regulatory requirements.
- Contributes to the development and implementation of procedures and protocols that enhance the delivery of comprehensive health services and quality assurance.
- Provides inpatient and on-call coverage as designated by the Medical Director.
- Monitors and manages chronic conditions, ensuring both symptomatic and well patients receive appropriate care.
- Coordinates the preparation of necessary reports and documentation, including medical records, as required by the department and regulatory agencies.
- Supervises nursing staff and assistants in examination settings and may serve as a mentor for team members advancing in their medical roles.
- May offer consultative services for complex diagnostic or therapeutic issues upon request from fellow physicians.
- Maintains and updates departmental policies, objectives, quality assurance programs, and safety standards as necessary.
- Promotes professional development through participation in educational programs, workshops, and meetings, staying informed on literature relevant to primary care.
- Attends required meetings and engages in committees as directed.
- Performs other duties as assigned.
Education:
Minimum:
- Graduation from an accredited medical school.
- Current California MD and DEA licenses.
- BLS certification.
- Board certification or eligibility in a primary care specialty.
- ACLS certification.
- Completion of clinical training.
- One year of experience post-clinical training, preferably in a community health setting.
- Experience supervising nursing staff.
- Bilingual (English/Spanish) preferred.
- Strong oral and written communication abilities.
- Preferred skills include skin biopsy, excision of skin lesions, suturing, casting, and phlebotomy.
- Must obtain staff privileges at local hospitals.
- Requires prolonged periods of standing and walking.
- Frequent sitting and assisting patients as needed.
- May require work at various sites and participation in on-call duties, including evenings and weekends.
- Pre-employment requirements include I-9 verification, physical examination, background checks, application completion, and orientation.
