Registered Nurse Case Coordinator

The Case Coordinator plays a crucial role in ensuring that patient care is effectively organized and managed. This position involves overseeing staffing and scheduling visits for skilled nursing (SN), medical social workers (MSW), and certified home health aides (CHHA). Responsibilities include:
- Conducting follow-up calls with patients after the start of care (SOC), recertification (RECERT), and resumption of care (ROC).
- Assigning supervisory visits for licensed vocational nurses (LVN) and CHHA.
- Ensuring timely submission of OASIS assessments and visit documentation.
- Coordinating with laboratories, pharmacies, and medical staff to track and obtain authorizations for health maintenance organizations (HMO).
- Managing durable medical equipment (DME) orders and supplies.
- Ensuring the presence of a CHHA care plan when applicable.
- Notifying visiting staff when a patient is transferred to a hospital or skilled nursing facility after ROC.
Qualifications:
- Minimum of one (1) year of experience in home health or case management.
- Ability to pass a background check.
- Eligibility to work in the United States.
- Proficiency in Microsoft Office applications, including Excel and Word.
- Strong attention to detail and effective note-taking skills.
- Excellent organizational abilities.
- Positive attitude and commitment to team success.
- Willingness to engage in ongoing training and skill development.
- Availability for on-call duties with a company-provided cell phone during after-hours (rotational).
The selected candidate must possess clear communication skills in English, with bilingual abilities considered an advantage. Effective communication via email and text is essential.
