About the Role

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in providing high-quality care to clients in their homes.

Responsibilities
  • Conduct comprehensive assessments and develop individualized care plans to address clients' physical therapy needs.
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to promote clients' independence and mobility.
  • Collaborate with nursing and other healthcare professionals to ensure seamless care coordination.
  • Educate clients and their families on rehabilitative care and activities to promote healthy living.
  • Accurately document client care management and services provided.
Requirements
  • Physical Therapist license.
  • Minimum one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
