Advanced Nursing Role at Catholic Medical Center

Catholic Medical Center seeks an experienced Advanced Practice Registered Nurse (APRN) to join our Mobile Community Health Team (MCHT) / Health Care for the Homeless Program (HCH). As a key member of our healthcare team, you will provide high-quality patient care and services to individuals and families in need.

This is a per diem opportunity offering the flexibility to choose your own schedule. We offer a competitive salary range of $85,000 - $110,000 per year, commensurate with experience.

About the Role:
  • Deliver patient-centered care to diverse populations, prioritizing their physical, emotional, and social needs.
  • Assess, plan, implement, and evaluate patient care plans in collaboration with other healthcare professionals.
  • Provide education and support to patients and their families, empowering them to take an active role in their healthcare.
Qualifications:
  • Bachelor's degree in Nursing or related field; Master's degree preferred.
  • Two years of experience as an APRN, with a focus on community health or homeless medicine.
  • Licensed by the state of New Hampshire as an Adult practice APRN.
  • ACLS certification desirable.
