Job Summary

We are seeking a highly skilled and compassionate Housing Stabilization Specialist to join our team at MySkye Care. As a key member of our housing services team, you will play a vital role in supporting individuals with disabilities to live successfully in the community.

Key Responsibilities
  • Provide community-based housing stabilization services to eligible adult clients with disabilities, focusing on transition and sustaining services.
  • Maintain a caseload of clients and conduct regular meetings to assess their needs and develop personalized housing plans.
  • Identify and assist with resolving barriers to accessing housing, including researching and contacting prospective housing options and supporting clients with lease negotiations.
  • Support clients in applying for benefits and accessing community resources to maintain housing stability.
  • Collaborate with clients to develop skills and strategies for maintaining housing, including training and coaching on being a good tenant and household management.
  • Participate in supervision and contribute to the development of program and client transportation plans as needed.
  • Maintain accurate billing and documentation in accordance with Department of Human Services guidelines.
Requirements
  • Bachelor's degree in social sciences, such as Social Work, Psychology, or Human Services.
  • At least one year of experience working in a community setting providing similar services.
  • Valid Minnesota driver's license, reliable transportation, and valid insurance.
  • Ability to work independently and as part of a team, with strong observation, assessment, and crisis management skills.
  • Excellent organizational and documentation skills, with proficiency in Microsoft Office suite and internet software.
