About the RoleThe Licensed Practical Nurse working in our Transitional Care Unit is responsible for providing direct care to adult and geriatric patients experiencing prolonged general medical or post-operative recovery. This role requires a high level of compassion, critical thinking, and effective communication skills.
Responsibilities- Provide medication administration, dressing changes, and vital sign monitoring to ensure optimal patient care.
- Document patient information accurately and thoroughly in the medical record.
- Collaborate with interdisciplinary teams to develop and implement individualized patient plans.
- Support and educate patients and families to promote successful transitions home.
- Education: Graduate of an accredited practical nursing program.
- Experience: Previous nursing experience preferred, but not required.
- Skills: Excellent verbal and written communication skills, effective knowledge of scope of practice, valuable time management skills, strong critical thinking/problem solving skills, and flexibility to work in a multi-tasking environment.
- Licensure/Certification/Registration: Active LPN license and IV Certification in the state of Missouri.
