Job Summary
We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments of patients in their homes, identifying potential health risks and developing personalized care plans to address these needs.
You will work collaboratively with our team of clinicians and healthcare professionals to deliver high-quality care and improve health outcomes for our patients. This is a PRN/Part Time position, offering flexible scheduling and the opportunity to work in a variety of settings, including home visits, skilled facilities, and telehealth.
Responsibilities
As a Home Risk Assessment Specialist, you will be responsible for:
- Conducting comprehensive health assessments of patients in their homes, including medical history, diagnosis, and treatment
- Developing personalized care plans to address identified health risks and needs
- Collaborating with our team of clinicians and healthcare professionals to deliver high-quality care and improve health outcomes
- Providing education and support to patients and their families to promote healthy behaviors and self-management
- Documenting patient information and care plans in accordance with Matrix Medical Network's policies and procedures
Requirements
To be considered for this role, you must have:
- A Master's degree in Nursing or a related field
- Current RN and NP licensure in the state of practice
- Board certification by a recognized nursing organization
- At least one year of experience as a Nurse Practitioner
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to work in a variety of settings, including home visits, skilled facilities, and telehealth
What We Offer
We offer a competitive compensation package, including a base rate of $90.00 or $100.00 (based on county) per completed assessment. We also offer flexible scheduling, mileage reimbursement, and the opportunity to work in a variety of settings.
