Hospice Care Specialist Job Description

The LPN Assistant Clinical Coordinator plays a vital role in supporting the Clinical Supervisor in scheduling staff and managing office functions. This position requires strong organizational skills and attention to detail, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:

  • Schedule CNA visits for the week in advance using the electronic health record system.
  • Verify physician orders and ensure accuracy before sending for signature.
  • Participate in the morning Access call in the absence of the Clinical Supervisor.
  • Assist with physician verbal orders, document, and track lab results.
  • Review and track lab results, fax to MD, and place copy in RN box.
  • Communicate patient information and changes to Nurse Triage via email.
  • Administer PPD or other vaccinations for staff as required.
  • Manage orders for equipment and supplies as needed.
  • Order IV meds and supplies from contracted vendors as required.
  • Assist with clinical issues when the Clinical Supervisor is out of the office.
  • Compile communication through telephone calls and contacts as requested by the Clinical Supervisor.
  • Assist with triaging nursing-related calls and dispatching staff.
  • Provide information to physicians, patients, family members, team members, and other callers.
  • Inform the Clinical Supervisor of unusual or potentially problematic patient/family issues.

Requirements:

  • High School Diploma or GED equivalent required.
  • Advanced education for clinical licensure; successful completion of IV therapy course (30 hours).
  • Minimum of 2-3 years nursing experience required, preferably in oncology, palliative care, geriatrics, or home health care.
  • Experience working with an interdisciplinary team preferred.
  • Current Cardiopulmonary Resuscitation (CPR) certification (American Heart Association).
  • Current driver's license and insurance.
  • Current, unrestricted LPN license in the state of Florida.
  • Expertise in various technologies such as Microsoft Office and electronic medical records.
  • Demonstrated ability to work non-judgmentally with persons of varied cultural, religious, and socioeconomic backgrounds and lifestyles.
