About the Role

We are seeking a compassionate and skilled Nurse Practitioner to join our team as a House Calls Nurse Practitioner. In this role, you will deliver evidence-based medical care to seniors and patients with complex needs in their residences.

Your responsibilities will include assessing, diagnosing, and treating a variety of health issues, as well as coordinating interdisciplinary care and managing patient records. You will have the opportunity to work independently while being part of a supportive team focused on improving patient quality of life.

Key Responsibilities
  • Provide comprehensive medical care to patients in their residences
  • Assess and diagnose various health conditions
  • Develop and implement personalized treatment plans
  • Coordinate with caregivers and other healthcare professionals
  • Manage patient records and ensure up-to-date documentation
Requirements

To be successful in this role, you will need:

  • An active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • A Master's degree in Nursing (MSN)
  • A minimum of five years of nursing experience; at least two years of nurse practitioner experience preferred
  • Strong interpersonal communication skills – verbal and written
  • Excellent time management skills with accountability for practitioner's daily workflow
Compensation and Benefits
