Job Summary:

The Clinical Social Worker LCSW PACE will be responsible for providing direct social work and case management services to participants of the MJH PACE Program. This includes collaborating with the interdisciplinary team to ensure effective care, participating in assessments and care planning, and referring participants to community agencies or facilities.

Key Responsibilities:
  • Collaborate with the interdisciplinary team to ensure effective care
  • Participate in assessments and care planning
  • Refer participants to community agencies or facilities
  • Consult and advise staff members on social, emotional, and cultural factors related to health and medical care
  • Participate in program and policy development of the Social Work component of Florida PACE Centers
Requirements:
  • Master's Degree from a school of Social Work accredited by the Council of Social Work Education
  • Two years' experience in case management or social work in a hospital, nursing home, or community-based program
  • Licensed Clinical Social Worker required
Preferred Qualifications:
  • Knowledge of geriatrics/gerontology
  • Bilingual in English/Spanish or English/Creole
