About the Role:

The HCBS Program Coordinator Assist position is an exciting opportunity to join our team at Hills & Dales Child Development Center. As a key member of our staff, you will play a vital role in helping to build meaningful lives for the adults we serve and enhancing the quality of their lives.

Key Responsibilities:

  • Oversee and manage the needs of our homes for people with disabilities, providing part oversight and part direct support.
  • Implement person-centered planning, individual goals, and self-assuring experiences in a manner consistent with our mission and philosophy.
  • Collaborate with our team to ensure the highest quality of care and support for our clients.
  • Develop and maintain relationships with clients, families, and community partners to ensure a seamless support system.
  • Stay up-to-date with relevant training and certifications, including CPR/First Aid, QBS, and Medication Manager training.

Requirements:

  • High school diploma or equivalent required.
  • Valid driver's license with proof of insurance (if required).
  • One year of direct support experience in a formal support provision setting and/or CNA preferred.
  • At least three months experience with HCBS Waiver Services for Supported Community Living preferred.
  • Must successfully complete Medication Manager and Child & Dependent Adult abuse training and maintain certification.
  • Continued required ongoing annual training as identified in Iowa Administrative Code.
