Job Title: Nurse Practitioner or Physician Assistant

Job Summary:

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team at a Skilled Nursing Facility in Laredo, Texas. As a key member of our primary care team, you will be responsible for managing a panel of SNF residents, focusing on treating patients in place in collaboration with a full primary care team.

Responsibilities:

  • Conduct comprehensive assessments and develop individualized plans of care for patients
  • Attend care team meetings and communicate with family members/caregivers to ensure the care team understands patients' goals and needs of care
  • Collaborate with physicians, care coordinators, facility staff, and other healthcare professionals to provide high-quality patient care
  • Perform direct care management support, including assessing and planning healthcare, reviewing and interpreting medical histories, and prescribing or performing various treatment modalities
  • Work collaboratively with a Clinical Manager and Clinical Director, as well as a local Medical Director

Requirements:

  • Texas NP or PA license
  • DEA
  • 2+ years advanced practice experience
  • 2+ years geriatric care experience (as APP)
