Job Summary

We are seeking a highly skilled Licensed Vocational Nurse to join our team at Focus Health. As a key member of our home health care team, you will provide high-quality patient care and support to individuals in their homes.

Responsibilities
  • Provide direct patient care and support, including administering medications, wound care, and vital sign monitoring.
  • Conduct thorough assessments of patients' health status and develop individualized care plans.
  • Collaborate with healthcare teams to coordinate services and promote quality care.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
  • Document patient progress and communicate changes in condition to healthcare providers.
Qualifications
  • Graduation from an accredited nursing program and current state licensure as a Licensed Vocational/Practical Nurse.
  • At least one year of clinical nursing experience, with home health experience preferred.
  • Valid driver's license and automobile insurance.
  • Ability to make home visits and provide skilled nursing assessments and care in the home.
