Job Summary

We are seeking a highly skilled Mental Health Clinician to join our team at Ft Lyons. The successful candidate will work directly with patients in our Supportive Residential Community, providing comprehensive clinical services and support.

  • Key Responsibilities:
  • Conduct psychosocial assessments and intake screenings to diagnose behavioral health issues.
  • Develop individualized treatment plans that address clients' specific needs.
  • Provide therapy, education, and consultation services to clients.
  • Work collaboratively as part of a multi-disciplinary team to ensure effective care.
  • Assist clients in accessing goods and services, including healthcare, housing, and income maintenance benefits.
  • Document all clinical services, telephone communications, and other interactions in a timely and complete manner.
  • Facilitate client connections with other departments and service agencies.
Requirements
  • Qualifications:
  • Master's degree in social work, counseling, or a related field; licensed LCSW or LPC with a CAC II or higher strongly preferred.
  • Valid driver's license required.
  • Previous experience working with the homeless population or in a related direct human service field is preferred.
