About the Role:

We are seeking a skilled Occupational Therapist to join our team at UP Health System Home Care & Hospice. As an Occupational Therapist, you will play a vital role in helping patients regain their independence and improve their quality of life.

Key Responsibilities:

  • Evaluate patients' functional status and occupational therapy needs, developing personalized plans of care to address their unique challenges.
  • Provide evidence-based occupational therapy services, working collaboratively with other healthcare professionals to ensure comprehensive care.
  • Conduct home assessments and adaptive equipment evaluations to identify opportunities for improvement and implement solutions.
  • Document patient progress and communicate effectively with healthcare teams to ensure seamless care coordination.

Requirements:

  • Current Occupational Therapy licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
