Nurse Care Specialist Job Description

Key Responsibilities:
  • Develop and implement individualized care plans in collaboration with healthcare professionals
  • Provide high-quality nursing care to individuals with developmental disabilities
  • Maintain accurate and up-to-date records of patient care
  • Collaborate with healthcare professionals to ensure comprehensive care
  • Ensure compliance with state and federal regulations

Requirements:
  • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
  • Ability to read, write, and follow oral and written directions
  • Ability to relate positively and favorably to individuals and families
  • Knowledge of individual service plans and related experience working with individuals with developmental disabilities
