Key Responsibilities- Monitor and evaluate prescribed therapies to promote optimal drug use and evidence-based practices.
- Provide medication information and consultation on pharmacokinetics, dose adjustments, and orders laboratory tests.
- Collaborate with pharmacy team, nurses, physicians, and other facility staff to resolve conflicts and ensure seamless care.
- Bachelor of Science or Doctorate degree in Pharmacy from an ACPE-accredited School of Pharmacy.
- Active pharmacist license in good standing with the State Board of Pharmacy.
- One-year acute care hospital experience.
