Caregiver Coordinator

We are seeking a qualified Registered Nurse to join our team as a Nurse Specialist. In this role, you will work closely with our clients to assess their medical and personal care needs, developing customized care plans to ensure their safety and well-being.
Your primary responsibilities will include conducting initial in-person assessments, followed by alternating monthly assessments, either in-person or virtually. You will document findings, create care plans, and communicate with clients' families and care teams to ensure understanding of care plans.
To succeed in this role, you will need a valid Registered Nurse (RN) license in the state of New Jersey, at least 2 years of nursing experience, preferably in home health care or geriatrics, strong communication and documentation skills, and the ability to conduct virtual assessments and manage telehealth technology.
We offer a competitive salary, with estimated earnings ranging from $45-$50 per in-person assessment and $35-$40 per virtual assessment. This position allows for flexible scheduling based on client needs and your availability.
