About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Home Health Nurse. As a key member of our care team, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and well-being.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical, emotional, and social needs, developing and implementing individualized care plans to address these needs.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
  • Document patient progress, including assessments, care plans, and outcomes, in accordance with regulatory requirements and company policies.
  • Participate in ongoing education and training to stay current with best practices, regulatory requirements, and company policies.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum of one year of nursing experience, preferably in home health or a related setting.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills, with ability to work independently and as part of a team.

As a valued member of our team, you will enjoy a competitive compensation package, including a salary range of $69,800 - $96,200 per year, based on experience and qualifications.
