The Clinical Nurse Specialist will oversee the coordination of all systems and services essential for a structured, multidisciplinary, patient-centered care team approach, ensuring high-quality and cost-effective care for the designated patient population.
Responsible for managing the treatment trajectory of patients, the specialist will collaborate with physicians, nurses, and other healthcare personnel to guarantee optimal patient outcomes are achieved within set timelines while utilizing resources efficiently.
Conduct comprehensive initial and ongoing evaluations, initiate disease management protocols, assess and manage outcomes, and ensure continuity of care through effective discharge planning, resource utilization, and variance analysis.
Serve as a primary contact for patients, families, healthcare team members, community resources, and staff as necessary. Ensure compliance with hospital and departmental policies and procedures. Patient care assignments may encompass neonate, pediatric, adolescent, adult, and geriatric age groups.PATIENT SAFETY: Adhere to patient safety-related policies, procedures, and protocols
* Document and communicate clinical findings
- PATIENT CARE: Develop treatment plans; provide and coordinate nursing care for assigned patients; may facilitate group therapy and/or educational sessions.
- VARIANCE: Review deviations from standardized care protocols with healthcare team members and implement resolution strategies.
- REPORTS & RECORDS: Maintain a computer-based tracking system and compile necessary reports and records.
- PATIENT SAFETY: Demonstrate a proactive approach to patient safety by identifying opportunities for improvement in current policies and processes.
- QUALITY: Engage in continuous quality improvement initiatives by evaluating patient care systems, including standards, protocols, and documentation.
- COMMITTEES: Attend meetings and represent the department or hospitals in related committees or community activities, as assigned by the supervisor.
- INFORMATION: Assist in developing activities and methods to ensure information is effectively communicated to appropriate members of the healthcare team.
- DATA: Conduct assessments, collect data, and analyze information in collaboration with patients, families, significant others, healthcare team members, and other relevant parties.
- PATIENT SAFETY: Identify and report/correct environmental conditions or situations that may pose undue risk to patients.
- COMMUNICATION: Utilize interpersonal communication strategies with individuals and groups of patients, families, significant others, and staff to achieve desired outcomes and satisfaction.
- IDENTIFICATION: Identify patients within designated specialty areas requiring case management interventions using established procedures, including census review, risk screens, and referrals.
- GOALS: Collaborate with patients, families, and significant others to establish patient-centered goals in conjunction with physicians, staff RNs, and other healthcare team members.
- PLAN OF CARE: Develop a comprehensive multidisciplinary plan of care effectively utilizing available tools and resources.
- RESOURCES: Coordinate and evaluate the use of resources and services in a quality-conscious, cost-effective manner, collaborating with appropriate providers to ensure effective, quality outcomes.
- DISCHARGE PLANNING: Conduct timely discharge planning by anticipating patient needs in collaboration with physicians, staff RNs, and other healthcare team members.
- INTERVENTIONS: Monitor and evaluate both short-term and long-term patient responses to interventions in collaboration with quality assurance and utilization review, maintaining necessary follow-up.
- CONTINUITY OF CARE: Collaborate with the healthcare team to ensure continuity of patient care across all healthcare settings; promote effective communication among team members, including patients, families, and significant others.
- CARE PLAN: Integrate recommendations and/or services from interdisciplinary team members into the care plan.
- COLLABORATION: Foster collaborative relationships with other departments/services and community healthcare agencies to facilitate and support quality care in the area of clinical expertise; act as a resource for complex patient care activities.
- DOCUMENTATION: Provide timely verbal and written documentation regarding the initial assessment and patient progress to other healthcare team members.
- ORIENTATION: Participate in the orientation and continuing education of staff RNs and other healthcare team members as appropriate.
- NEEDS: Assess the needs of patients/families/significant others concerning medical diagnoses, treatment, and resources; provide treatment options, financial resources, psychosocial support, and discharge planning in collaboration with relevant resources.
- ORDERS AND REFERRALS: Obtain necessary orders from physicians to initiate home health referrals, home infusion medications and supplies, oxygen, and equipment; coordinate referrals for oxygen and equipment.
- PATIENT SAFETY: Report potential or actual patient safety concerns, medical errors, and/or near misses promptly.
- PATIENT SAFETY: Encourage patients to actively engage in their care by asking questions and reporting any treatment or situations that they do not understand or may seem inappropriate.
- PATIENT CENTERED MED: Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicable.
Bachelor's Degree
