Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at Adoration Home Health and Hospice. As a LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

About the Role:
  • Provide routine care and general observation of clients' health status
  • Assist the RN and physician in treatments and communicate with clients and their families/guardians
  • Report changes in condition and/or additional client needs to supervisor
Responsibilities:
  • Provide 1:1 care for clients following their plan of care at all times while maintaining their dignity and independence
  • Monitor clients' vital statistics and report abnormal findings to the Case Manager and Physician
  • Administer medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
  • Complete routines as established by other care professionals such as PT, OT, Speech Therapists
  • Ensure infection control policies are maintained at all times, reporting possible communicable diseases or infections and ensuring the overall implementation and practice of infection control procedures are implemented
Qualifications:
  • LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
  • Valid driver's license, acceptable driving record and proof of car insurance
  • CPR professional certification
  • Experience in administering medications
  • 1+ years of nursing experience preferred
  • Home health care experience preferred
