About the Role
">The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively with clients, families, and caregivers. They will also possess strong problem-solving skills and be able to develop and implement effective service plans.
">Responsibilities
">- Assess client needs and develop personalized service plans to support their independence and well-being.
- Collaborate with clients, families, and caregivers to identify and address challenges and goals.
- Coordinate services and referrals to ensure seamless support for clients.
- Maintain accurate and up-to-date records of client interactions, services provided, and outcomes.
- Participate in interdisciplinary teams to share knowledge and best practices.
- Represent LifePath Inc. in a professional manner and promote the organization's mission and values.
Requirements
">- Bachelor's degree in a relevant field (e.g., social work, healthcare, or related field).
- Previous experience in health or human services, preferably in case management or a related role.
- Valid driver's license and reliable vehicle for travel within the service area.
- Strong written and verbal communication skills, with the ability to communicate effectively with diverse stakeholders.
- Ability to work independently and collaboratively as part of a team.
- Flexibility to adapt to changing program demands and priorities.
