We are seeking a highly skilled and compassionate Physical Therapist to join our team at Life Care Center of West Bridgewater. As a Physical Therapist, you will play a vital role in helping our patients achieve their rehabilitation goals and regain their independence.
About the RoleThe successful candidate will be responsible for providing direct rehabilitation care to patients in physical therapy, working closely with our interdisciplinary team to develop and implement individualized treatment plans. You will also be responsible for evaluating patient progress, modifying treatment plans as needed, and communicating effectively with patients, families, and healthcare professionals.
Responsibilities- Provide direct rehabilitation care to patients in physical therapy, including evaluations, treatments, and progress assessments
- Develop and implement individualized treatment plans in collaboration with the interdisciplinary team
- Evaluate patient progress and modify treatment plans as needed
- Communicate effectively with patients, families, and healthcare professionals
- Participate in community health matters and projects as appropriate
- Stay up-to-date with industry developments and best practices in physical therapy
- Graduate of an accredited program in physical therapy (BSPT, MSPT, or DPT)
- Current registration/licensure in applicable State
- One (1) year experience in post-acute care or related setting preferred
- CPR certification upon hire or obtain during orientation
