Pediatric Pharmacist Job Description

We are seeking a highly motivated and experienced Pediatric Pharmacist to join our team at UNC Health Care. As a Pediatric Pharmacist, you will play a critical role in providing patient-centered care to our pediatric patients.

Responsibilities:
  • Ensure timely medication access for pediatric patients, including order review and verification.
  • Clarify incomplete, inaccurate, or contraindicated prescriptions with prescribers, nurses, or other healthcare professionals.
  • Identify and intervene on problems related to medication needs of pediatric patients.
  • Utilize and analyze patient data to provide optimal drug therapy recommendations.
  • Assess pediatric patients' medication therapy regimens and make appropriate clinical interventions with providers.
  • Perform protocol-driven clinical services, such as IV to oral dosing, stewardship activities, and therapeutic dose monitoring.
  • Compound and oversee the preparation and dispensing of medications, including sterile products.
  • Maintain patient profiles within electronic medical records to facilitate communication.
  • Comply with medication formulary, therapeutic substitution, and other programs designed to optimize drug therapy while minimizing cost.
  • Facilitate discharge and transfer of pediatric patients, including medication reconciliation and discharge counseling.
  • Practice medication safety, including adverse drug event and medication error reporting.
  • Participate in Code Blue situations for pediatric patients.
  • Precept students, residents, and participate in training new pharmacy staff.
Requirements:
  • PharmD or BS Pharmacy Degree.
  • Active pharmacist licensure with the NC Board of Pharmacy or obtainable before initiation of employment.
  • Completion of a PGY1 or PGY2 residency in pediatrics or an equivalent level of pharmacy experience.
