Job Summary

Element Care is seeking a skilled Registered Nurse to join our PACE Interdisciplinary Team. As a Geriatric Care Specialist, you will play a vital role in providing high-quality care to our frail elderly participants.

Key Responsibilities
  • Direct Care Provision: Provide comprehensive nursing care to participants, including initial and periodic assessments, care plan development, and implementation.
  • Interdisciplinary Collaboration: Work closely with the PACE team to develop and revise care plans, ensuring comprehensive decisions are made to ensure participant safety and well-being.
  • Quality Assurance: Monitor clinic care and service delivery to ensure conformance with established quality assurance and operational policies, procedures, and standards.
  • Health Status Monitoring: Remain attentive to participants' health status and social functioning, identifying overt problems/needs and setting priorities.
  • Care Plan Development: Coordinate with the interdisciplinary team to develop comprehensive care plans for each participant, incorporating immediate and continuing care needs.
  • Medication Management: Ensure all tests, lab work, and diagnostic studies ordered by the physician have been carried out, and coordinate medication delivery to participants' homes.
  • Education and Consultation: Provide education and consultation to participants and caregivers regarding care plans, nursing treatments, and medications.
  • Communication: Communicate participants' conditions to primary caregivers, physicians, and the multidisciplinary team.
  • Relationship Building: Build strong relationships with participants, caregivers, families, and community providers to facilitate optimal care and financial oversight.
  • Additional Responsibilities: Participate in after-hour visits, and perform other duties as required.
Requirements
  • License and Certification: Registered Nurse currently licensed in the Commonwealth of MA.
  • Experience: Minimum of two (2) years experience with the geriatric population.
  • Technical Skills: Proficient in the use of Microsoft Office tools.
  • Travel and Transportation: Ability to travel locally on a frequent basis in a personal vehicle.
