Position Overview

Polaris Healthcare is seeking a dedicated Registered Nurse for a Per Diem role.

Key Responsibilities
  • Conduct in-person assessments for adult patients within our facilities.
  • Perform and document UAS assessments, ensuring compliance with patient care standards.
  • Complete necessary paperwork, with the possibility of remote documentation.
Qualifications
  • Must possess a valid NYS Registered Nurse license.
  • Recent experience in UAS assessments is essential, preferably in an assisted living environment.
  • Familiarity with 4449c, MD orders, and interim order documentation is preferred.
  • Experience in assisted living settings is strongly preferred.
  • Knowledge of UAS assessments related to RUG scores in physical, occupational, and speech therapy is highly desirable.
  • Experience with OASIS assessments is a plus but not mandatory.
Work Environment

This role requires flexibility to travel between facilities as needed, ensuring high-quality care for our residents.
