Nurse Navigator

SSM Health is seeking a skilled Nurse Navigator to provide exceptional patient care and support throughout the continuum of care in a clinic setting. As a key member of our multidisciplinary team, you will work closely with physicians, advanced practice providers, and other healthcare professionals to ensure quality outcomes and promote patient-centered care.
Responsibilities- Collaborate with healthcare teams to develop and implement patient care plans that address physical, emotional, and social needs.
- Provide education and support to patients and their families on disease management, treatment options, and self-care strategies.
- Coordinate referrals and follow-up care for patients who require additional services or support.
- Utilize clinical expertise to identify and address potential barriers to care, such as social determinants of health and healthcare access.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
- Graduate of an accredited school of nursing with a current RN license in good standing.
- Minimum of 3 years of experience in a clinical setting, preferably in a patient navigation or care coordination role.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and healthcare teams.
- Strong analytical and problem-solving skills, with the ability to navigate complex healthcare systems and policies.
