Key Responsibilities- Assess patient's physiologic health status and develop a plan of care to address their needs.
- Collaborate with physicians and other healthcare professionals to coordinate patient care and treatment.
- Conduct patient interviews, explain policies and procedures, and review patient charts to ensure accurate and effective care.
- Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status and adjust the plan of care accordingly.
- Communicate and document patient's physiologic health status and plan of care.
- Elicit patient's perception of medical/nursing care and expectations, and identify coping mechanisms, knowledge level, and ability to comprehend.
- Develop criteria for measurement of patient outcomes, identify actual/potential patient problems, and develop patient outcome statements and individualized patient goals.
- Formulate the plan of care, establish priorities, and coordinate the cost-effective use of supplies, equipment, and medication.
- Implement the plan of care, maintain constant vigilance over patient care, and ensure safety precautions are followed.
- Administer medications correctly and safely, and document medication administration.
- Participate in patient and family education, provide explanations of patient condition, and communicate assessment data to care team members and appropriate agencies.
- Graduate from an accredited School of Nursing.
- Current license to practice as a Registered Professional Nurse in New York State.
- BCLS and ACLS certification preferred.
