As an Inpatient Pharmacist with Kaiser Permanente, you will play a critical role in ensuring the safe and effective use of medications for our patients. Under indirect supervision, you will consult with patients and medical personnel to optimize medication therapy, dispense compounds, procure, store, and distribute pharmacy products, and provide medical personnel and patients with medication information and product identification.
Key Responsibilities:- Monitor and manage medication therapy for hospitalized or ambulatory patients to ensure optimal therapeutic results and cost-effective prescribing.
- Review laboratory test results and patient progress to detect adverse drug reactions and confer with medical personnel on laboratory test interpretation, patient care, and treatment.
- Interview patients to obtain information on medication use, allergies, and sensitivities, and document this information in the patient's electronic medical record.
- Advise patients on medication directions, storage requirements, importance of compliance, precautions, and warnings.
- Provide medical personnel with medication information, drug therapy options, and pharmacokinetic dosing advice.
- Order and adjust dosages, order laboratory tests, and administer medication under protocol according to established policies and procedures.
- Dispense, compound, procure, store, and distribute pharmacy products, and monitor the effectiveness and accuracy of medication distribution.
- Participate in education and training programs, and provide inservices and educational programs for pharmacy, medical personnel, and patients as needed.
- Bachelor's degree in Pharmacy or Pharm.D. required.
- Pharmacist License (California) within 1 month of hire.
- Basic Life Support certification required.
- Comprehensive knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathologies, disease states, and related information for a medical management plan.
- Knowledge of aseptic technique, and the preparation of sterile IV products and chemotherapeutic agents.
- Possess excellent verbal and written communication skills.
- Ability to operate the pharmacy computer system(s) and other equipment.
