About the Role

VNS Health is seeking a skilled Registered Nurse to join our Home Health team in Union City, New Jersey.

This is a unique opportunity to redefine the standard of patient-centered care for New Yorkers, keeping them out of the crowded hospital system and allowing them to heal and age in the comfort of their own homes and community.

As a Home Health RN, you will provide individualized care plans and exceptional clinical outcomes to our neighbors in need, working collaboratively with an interdisciplinary care team to deliver compassionate care and unparalleled expertise.

Responsibilities
  • Practice independently in the community, delivering personalized nursing and care management to patients in their homes or care facilities.
  • Constantly evaluate evolving patient needs and respond with plan of care adjustments.
  • Collaborate with the care team to design and deliver individualized care plans.
  • Current license to practice as a Registered Nurse in New York State.
  • Minimum of one year nursing experience in a medical/surgical environment.
