We are seeking an experienced Primary Care Physician to join our team at CenterWell Senior Primary Care. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care to seniors in a value-based care setting.
About the RoleYou will work closely with our team to evaluate and treat patients, monitor their care, and coordinate services as needed. Your expertise and guidance will help us deliver exceptional patient outcomes and improve quality of life for our patients.
Responsibilities:- Act as a lead in our team-based care environment.
- Evaluate and treat patients in accordance with established standards of care.
- Monitor patient care using available data and chart reviews.
- Coordinate patient services, including specialty referrals, hospital admissions, and durable medical equipment.
- Current and unrestricted medical license.
- Graduate of an accredited MD or DO program.
- Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine is preferred.
- Total compensation package could exceed $300K depending on experience and qualifications.
