Responsibilities
  • Assess patient needs and develop personalized care plans to address physical, emotional, and financial challenges.
  • Collaborate with patients, families, and healthcare providers to ensure comprehensive care and support.
  • Provide education and guidance on disease management, treatment options, and available resources.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as needed.
  • Develop and maintain strong relationships with patients, families, and healthcare providers to ensure seamless care coordination.
Requirements
  • Master's degree in Social Work (MSW) from an accredited institution.
  • Current state licensure (e.g. LCSW, LMSW) and meet all state regulations for practice in a dialysis setting.
  • At least two years of clinical experience, preferably in an ESRD setting.
  • Current CPR certification preferred.
  • Excellent communication and interpersonal skills.
