We are seeking a skilled Licensed Vocational Nurse to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities- Provide professional nursing services under the direction of a RN, adhering to state and federal regulations, and CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans to address patients' rehabilitative needs, preventative care, and illness management.
- Maintain open communication with patients, physicians, and healthcare teams to ensure seamless care coordination and discharge planning.
- Document patient progress, maintain accurate records, and submit reports as required.
- Collaborate with multidisciplinary teams to provide comprehensive care and support to patients and their families.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
