Job DescriptionThis role involves delivering comprehensive primary care and evaluating acute illnesses. The ideal candidate will have experience working with frail, elderly patients, with a strong preference for backgrounds in value-based care, PACE, or primary care.
Responsibilities- Assess Patients: Conduct in-person assessments and create care plans with the Interdisciplinary Team (IDT).
- Supervise Staff: Oversee 2-3 Nurse Practitioners and complex cases.
- Coordinate Care: Collaborate with Community-Based Primary Care Physicians and the IDT to ensure coordinated care.
- Develop Best Practices: Review outcomes data, participate in quality improvement initiatives, and develop best practices.
- Education: Completion of an accredited residency program (Internal Medicine, Family Practice, Emergency Medicine, or Geriatrics preferred).
- Certifications: Board Certified with an active physician license in California. State CDS Certification, DEA Registration, and experience with Medicare and Medi-Cal preferred.
- Experience: Minimum of 2-3 years of experience, ideally in value-based care, primary care, or SNF settings.
- Professional Development: $2,500 annual CME reimbursement and 3 days of paid CME leave.
- Comprehensive Benefits: Medical, dental, vision insurance, 401(k) with match, malpractice and tail insurance.
- Work-Life Balance: 17 days of PTO, 12 holidays, and sick time.

$180,000 per year.
