Job Overview

Position Title: Paraprofessional II / Case Management Specialist

Program Focus: Children's Services – Intensive In-Home / Intensive Independent Living Skills

Position Summary:

This role is dedicated to providing Intensive In-Home and Intensive Independent Living Skills services, along with Case Management for children facing significant emotional and behavioral challenges. The incumbent will operate in alignment with the program and policy guidelines of the Children's Services Program at Hale Na`au Pono.

Reporting Structure:

Administrative: Reports to the Program Director, Children's Services.

Clinical: Collaborates with the assigned Therapist for the youth served.

Classification: Full-Time, Non-Exempt

Compensation: $17.57 per hour

Mission Statement:

Our mission is to offer children and youth with serious emotional disturbances a therapeutic family-based alternative to institutionalization, facilitating their return to and/or maintenance in their home and community.

Core Responsibilities:

  1. Administrative Duties:
  • Submit service charge tags promptly.
  • Receive daily and weekly assignments from the Program Director/Clinical Director.
  • Uphold confidentiality and adhere to ethical standards consistently.
  • Follow health and safety policies diligently.
Clinical Case Management Duties:
  • Engage as a member of a multidisciplinary team, implementing prescribed services.
  • Provide individualized companionship and skills training in home or community settings.
  • Assist with the admissions process, ensuring completion of necessary documentation.
  • Collaborate with other service providers for coordinated service planning.
  • Honor the cultural values of those served, aiming for positive outcomes.
  • Document interactions and maintain accurate case records as per policy.
  • Conduct regular face-to-face contacts in accordance with treatment plans.
  • Complete monthly case management reports as required.
  • Maintain compliance with quality assurance standards.
  • Be available for on-call crisis intervention and stabilization services.
  • Support experienced staff in conducting psycho-educational groups.
  • Participate in Treatment Team meetings with families and other professionals.
  • Facilitate the implementation of tasks defined in the Mental Health Treatment Plan.
  • Provide substantial time with youth and families as needed.
  • Utilize strength-based approaches to assist families in managing ongoing challenges.

Qualifications:

  1. Knowledge and Skills:
  • Understanding of community culture and lifestyle.
  • Adept at interpreting issues within a cultural framework.
  • Ability to apply cultural insights in interventions.
  • Possess personal qualities such as warmth, acceptance, sound judgment, emotional stability, and reliability.
  • Genuine desire to work with children facing emotional challenges.
Education and Experience:
  • Bachelor's degree in Social Work (BSW) with 2 years of relevant experience, or a high school diploma/GED with 4 years of case management experience, particularly with children.
  • Physical capability to perform job duties with or without reasonable accommodation.

Additional Requirements:

  1. Valid driver's license and a clean driving record.
  2. Willingness to work flexible hours, including nights and weekends.
  3. Consent to background checks and drug testing as per policy.
  4. Proof of full vaccination against COVID-19 and adherence to health protocols.
