Your role is crucial to our success. Each position significantly impacts our patients' lives, and you have the chance to make a meaningful difference. We are seeking a committed Pharmacist PRN to join our team.
Key Responsibilities and QualificationsAs a Pharmacist, your primary responsibilities will include monitoring, assessing, and recommending the appropriate and effective use of medications for patient care. You will engage with patients and healthcare professionals to ensure compliance with current standards of practice and maintain continuity of care to achieve optimal patient outcomes through safe, timely, and cost-effective pharmaceutical services.
- Interpret medication orders and accurately input them into the computer system.
- Provide effective medication therapy management.
- Compound and dispense pharmaceuticals with precision.
- Monitor drug therapy regimens for contraindications, interactions, allergies, and appropriateness of medication and dosage.
- Propose suitable, cost-effective therapeutic alternatives to medical staff.
- Maintain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges, and promoting rational drug therapy selection.
- Assist with clinical services such as drug consultations, nutritional support, pharmacokinetics, anticoagulant monitoring, pain management, and renal dosing.
- Complete reports regarding medication errors and adverse drug reactions.
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD).
- Preferred one year of experience in a hospital setting.
- Current Pharmacist license with the Texas State Board of Pharmacy.
- Sterile Products (IV) Certification through the Accreditation Council for Pharmacy Education (ACPE) within 60 days of hire.
