Position Summary:
The role of the Licensed Practical Nurse is pivotal in supporting the mission of NHC. This position exemplifies leadership and professionalism, representing the organization positively within both the medical community and the broader community.
Qualifications:
Health: Ability to meet performance requirements.
Education and Training:
Graduation from a nursing program accredited by the state is essential. The candidate must maintain a current, unencumbered license to practice in the state of employment. Continuous personal and professional development is encouraged, including participation in continuing education and contributing to the education of nursing partners.
Job Knowledge and Capabilities:
The LPN is responsible for maintaining clinical competency through the application of integrated nursing knowledge and skills, alongside effective leadership and communication abilities. Responsibilities include:
- Utilizing the nursing process for assessment, planning, and implementation of patient care.
- Exhibiting organizational skills to prioritize patient care needs.
- Integrating current standards of practice with local, state, and federal regulations.
- Engaging in continuous quality improvement and problem-solving.
Performance Requirements:
Physical Demands:
The role requires the ability to stand or walk for 8-12 hours daily, lift 60-70 lbs occasionally, and perform various physical tasks throughout the day.
Specific Duties and Responsibilities:
The LPN will work collaboratively with the interdisciplinary care team to ensure accurate patient assessments and the development of individualized care plans. Key responsibilities include:
- Maintaining open communication with patients and families.
- Monitoring patient conditions and ensuring appropriate nursing care is provided.
- Assisting with medication administration and treatments.
- Participating in the orientation and supervision of new nursing partners.
- Conducting weekly assessments and maintaining accurate documentation.
