Job Summary

We are seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to join our team. As a pediatric home health nurse, you will provide skilled nursing care to children in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities

  • Provide skilled nursing care to pediatric patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to address these needs.
  • Collaborate with our interdisciplinary team, including therapists, social workers, and other healthcare professionals, to ensure comprehensive care.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current on best practices in pediatric home care.

Requirements

  • RN/LVN with a current valid license to practice as issued by the State Board of Nursing.
  • Current CPR certification.
