Position Summary:
The Personal Care Assistant plays a vital role within the home care team, working under the guidance of a Registered Nurse or Therapist to deliver essential personal care services tailored to meet the needs of patients. This role involves observing patients, documenting care provided, and reporting observations to ensure quality and continuity of care while upholding the mission, vision, and values of VNA Health.

Key Responsibilities:
The duties of the Personal Care Assistant include, but are not limited to:

  1. Delivering personal care services such as:
    1. Bathing
    2. Back massages
    3. Oral hygiene assistance
    4. Shampooing
    5. Changing bed linens
    6. Helping patients with dressing
    7. Skin care to prevent breakdown
    8. Assisting with toileting needs
    9. Maintaining a clean and orderly living environment for patients
  2. Planning and preparing healthy meals.
  3. Assisting patients with feeding when necessary.
  4. Taking and recording vital signs as instructed (with demonstrated competency).
  5. Meeting daily productivity standards set by the organization.
  6. Understanding regulations related to home health service delivery.
  7. Performing basic therapeutic procedures as directed (with demonstrated competency).
  8. Assisting patients with self-administered medications.
  9. Managing laundry tasks for patients as needed.
  10. Ensuring patient safety and proper use of equipment.
  11. Reporting significant changes in patient conditions to the assigned nurse.
  12. Adhering to documentation and care standards set by the organization.
  13. Participating in required meetings and training sessions.
  14. This summary represents the primary responsibilities of the role; additional tasks may be assigned as necessary.

Qualifications:

  1. Active Home Health Aide Certification in California.
  2. Current CPR certification.
  3. Valid driver's license and insured vehicle.
  4. Ability to read and follow care instructions and document services provided.
  5. Self-motivated with the capacity to work independently.
  6. Empathy for the needs of patients who are ill, injured, or impaired.

Physical Requirements:

  1. Ability to lift up to 50 lbs.
  2. Ability to push/pull weights of 200 lbs.
  3. Ability to transfer individuals weighing up to 170 lbs.
  4. Capability to stoop, bend, and squat.
  5. Ability to navigate one flight of stairs.
  6. Ability to walk a mile.
  7. Ability to sit for a minimum of one hour.
  8. Visual and auditory acuity.
