Responsibilities- Assess and manage patients with complex chronic conditions, including those with acute and chronic health care needs.
- Collaborate with the interdisciplinary healthcare team to make diagnoses and develop individualized plans of care.
- Order and collect data using appropriate assessment techniques and diagnostic procedures.
- Prescribe diagnostic strategies and therapeutic interventions, including medications and procedures.
- Document patient progress and maintain accurate and timely records.
- Maintain ongoing communication and collaboration with the healthcare team, patients, and families.
- Master's Degree in Nursing or related field.
- National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant.
- BLS certification.
