M Health Fairview is seeking a Visit Facilitator to support in-person clinic visits and help guide patients through the care experience at the M Health Fairview Pain Management Center in Southdale, MN.
Key Responsibilities- Prepare patients for appointments by communicating with them prior to their scheduled visit to ensure medical records are collected and available.
- Assist in the rooming process by collecting and detailing pertinent information, including reason for visit, medication review, vitals collection, and health questionnaires.
- Support the patient connection to the provider for the visit and serve as a resource to ensure a positive patient/provider experience.
- Complete visit wrap-up and provide after-visit summaries to patients, including assisting with scheduling and coordinating follow-up appointments.
- Communicate efficiently with patients, providers, and care teams, demonstrating comfort with in-person and virtual technologies.
- At least 1 year of customer service experience.
- Proficient with technology, including computer hardware and programs, and smart devices such as phones or tablets.
- Ideal candidate has experience in customer service, medical terminology, and experience working in a healthcare setting.
- Successful completion of NA, EMT, or HUC program.
- Experience with medical terminology, Epic Health Records, and telephonic customer experience.
