Role OverviewWe are seeking an Advanced Practice Provider to join our inpatient Palliative Care Team at Chester County Hospital. As a key member of our interdisciplinary team, you will provide expert symptom management, psychosocial support, and advance care planning for patients and families facing serious illness.
Responsibilities- Consult with patients and families to provide personalized, comprehensive care plans that align with their goals
- Collaborate with the primary healthcare team to develop and implement effective treatment plans
- Participate in daily inter-professional palliative care team rounds to facilitate team communication and collaboration
- Provide expert symptom management and guide crucial discussions with patients and families
- Master's prepared Nurse Practitioner with Acute Care certification or Master's prepared Physician Assistant
- Current Pennsylvania license (or eligible to obtain)
- A minimum of 2-3 years of experience as an Advanced Practice Provider in the acute care setting is strongly preferred
- Training/experience in palliative care, hospice, or geriatrics is preferred but not required
