Key Responsibilities

The Patient Care Manager - Hospice collaborates with the Executive Administrator/Executive Director/Director of Clinical Operations to oversee all patient care functions, ensuring the provision of safe and effective care that complies with established nursing standards, regulatory guidelines, and payer requirements.
  • Accountable for adherence to hospice policies and procedures, as well as local, state, and federal regulations relevant to the assigned team.
  • Proactively advocates for and guides the assigned team on quality care and patient safety.
  • Engages in comprehensive problem-solving and complaint resolution.
  • Conducts training sessions for staff to enhance patient health and safety, familiarize them with regulatory matters, and address any identified educational needs.
  • A minimum of 4 years of full-time experience as a Registered Nurse or equivalent is required.
Qualifications

License Requirements
  • Current RN licensure in the state of practice along with one year of clinical experience.
  • Current CPR Certification.
  • Valid driver's license, vehicle insurance, and access to reliable transportation or public transit.
