About the Role

As a Certified Healthcare Navigator at TotalMed, you will play a crucial role in developing and implementing holistic care management activities to achieve quality and cost-effective healthcare outcomes.

Key Responsibilities

  • Evaluate member needs, resources, and recommend personalized plans for optimal outcomes
  • Collaborate with members, families, caregivers, and care providers to ensure accessible services
  • Provide resource support for various member needs and monitor care plans/outcomes
  • Interact with healthcare providers and partners to ensure member needs are met
  • Collect, document, and maintain member information and care management activities

Requirements

  • 2+ years of case management experience
  • Experience with electronic medical records
  • Strong computer skills (MS Office)
  • Bachelor's Degree
  • Bilingual in Spanish

Estimated Salary: $28.00 per hour
